Purchase Order (US Universities or Hospitals only)_______________

$13.00 for shipments of up to 6 vials of test strips.
$18.00 for shipment of 7-15 boxes of test strips.
$20.00 for shipment of Individual packages and 16-30 boxes of test strips. Also used for shipment of Analyzer with other products except Safety Lancets.
$25.00 for shipment of Small Team and Academic package.
$35.00 for shipment of Large Team package.
Home residence - add $2.50; Remote area - add $2.50; Western US - add $2.50 (Mountain and Pacific Time zones)
Other US orders - depends on costs.
International shipping estimates will be provided on request. To Canada it is usually $35-$80 depending upon weight and location. Generally it is $50-$90 to Western Europe depending upon the weight and destination. Shipment to other areas in Europe will be provided on request. Shipments to Asia, Australia, South America and Africa will be based on country and weight.
